Advance Directives: An Advance Health Care Directive is a form that helps others give you the care you would want when you cannot make decisions.  Other names for the form are “Special Power of Attorney for Health Care", “Durable Power of Attorney for Health Care,” or “Living Will.”

• Advance Directive form Print and fill out this form by hand.
• Advance Directive "fill-in" form Enter your information on the computer and then print the form.
• Advance Directive instructions Instructions for filling out your form.
• Spanish/English Advance Directive form A bilingual form for Spanish speakers.
