Forms for Admission Before your first appointment, please take time to fill out some information that will help us develop an appropriate treatment plan for you and your family. Please come a few minutes early to your appointment to complete the forms if you are unable to do this at home. All new clients should fill out a Social/Medical History form prior to their first appointment. The form is available in English or Espaņol. There is an adult version and a child/youth version that parents or guardians should fill out for their child. Choose a form to print by clicking the language and age you need:
